| Faculty Application Form If you would like to help out at the Jedi Academy at JEDI, you are welcome to apply to become part of the Faculty. In order to apply, please fill out all the information below. Any information you provide to us will be used soley for the purposes of faculty evaluation and will not be under any circumstances released or available to the public. To fill out the form and subit it to us, please copy and paste all the questions below into an E-mail, and send to academy@thejedi.org. Please put "Faculty Application Form" as the message heading. 1. What is your EZboard User Name? Online name? Real name (optional)? 2. What is your age? Date of birth? 3. What is your primary E-mail address? Secondary E-mail address (if applicable)? 4. What are your Instant Messenger Handles? (include MSN, AIM, Yahoo!, ICQ, or any other IM's) 5. Please list any previous or current positions you have held in relation to what we do here at the Academy. Have you held a Faculty position before? What website positions have you had in the past? 6. Please list any previous or current offline courses you have taught or are still teaching. They do not have to be governmental in nature (i.e. Yoga class, Meditation, Graphic Arts, College level philosophy, etc.). What are they about? What did you learn (or are learning) from the experience? 7. What is your educational background? Along with any educational degrees or certificates you may have earned, please list any post-graduate courses you have taken and passed (or are currently taking) in relation to our academic field of interests. Please also include a brief synopsis (one or two sentances) of each course you mention. 8. How would you describe your personality? How would you describe your work ethic? 9. Is there any specific position(s) (whether it exists now or not) you would like to fill for the Faculty? Please be as specific and detailed as possible. Why would you want to fill the position(s)? 10. What knowledge, skill, and/or experience can you list to help us in evaluating your application form? Please make this as detailed as you can. 11.
